An electronic form I-9 must be completed prior to orientation. When attending Orientation, you must bring proof of eligibility to work in the United States. Two forms of local and/or federal issued identification are required. They must be ORIGINAL documents; copies will not be accepted. Please review the list of acceptable documents.

For ECU Health Corporate, ECU Health Medical Center, Greenville Based ECU Health Physicians staff and ECU Health Community Hospital new hires:
You completed your Form I-9 online as part of your pre-boarding process.  You still must bring your original document(s) to orientation.

For ECU Health Physicians providers:
Log in to Employee Self Service with the E# and password provided in your Orientation Letter to access and complete your form I-9.  (See I-9 instructions.)
